What’s the Incentive Program?
It’s simple - we’ll reward you for taking care of yourself! Complete your Annual Preventive Physical, Well-being Assessment, and other healthy activities to earn rewards.
Employees can earn $400 | Eligible spouses/domestic partners can earn $200
The 2024 Well ALL Ways Program and incentive window is closed. 2024 Preventive Physical Forms are no longer being accepted. Participants are able to submit 2025 Preventive Physical Forms at the link below beginning on 1/24/2025.
Follow the steps below :
1.) Sign up for an account on the Well ALL Ways Portal. To access the Well ALL Ways Portal, select either the employee or spouse/domestic partner button above.
2.) Log in to the Portal and take the Well-being Assessment.
3.) Schedule your preventive physical with your primary care provider.
4.) Print the 2025 Preventive Physical Form to take to your appointment.
5.) Submit your completed Preventive Physical Form.
- Employees and eligible spouses/domestic partners must have an active account on the portal to be eligible for any 2025 incentives.
