If you are a Marathon Retiree with Level Premium Life Insurance coverage, you can access beneficiary information via Prudential’s online beneficiary management system.

Retirees can access the web at any time to make a new designation, change, or view an existing designation.

To access your beneficiary information through the internet go to the following website: https://giselfservice.prudential.com.

1. Select First Time User from the right of the screen.
2. In the Control Number field, enter 44076.
3. In the ID Field, enter your social security number without the dashes.
4. Enter your Last Name in the designated field and select Continue.
5. At the next screen, you will be prompted to create a personal ID and personal password. Enter a User ID and Password of your choice. This User ID and Password will be your permanent individual identifier, so you should retain this with your records. Complete the required information and select Submit.
6. You will receive a Transaction Successful message and then you can select Return to Login.
7. Enter your newly created User ID and password and select Submit.

NOTE: Be sure to SUBMIT your beneficiary designations at the conclusion of your transaction.

• If you don’t have access to the internet, not to worry! You can designate your beneficiary via a paper form. Just complete the form and mail to the address indicated at the top of the form.
